Heart failure

Sandwell Community Heart Failure Service provides a specialist nursing service. This is for patients with chronic heart failure who are registered with a Sandwell GP.

The service is provided by six specialist heart failure nurses, along with a healthcare assistant, a palliative care heart failure nurse and a business support officer.

Two members of the team are sponsored and supported by the British Heart Foundation.


We provide specialist nursing for the management, support and education of patients with chronic heart failure. We work closely with cardiologists (heart specialists) and lead physicians, treating patients at all stages of coping with heart failure.

We care for patients with a combination of home visits and telephone contact. We also provide community clinics where patients are seen in a general practice or health centre by a specialist nurse.

We assess patients and help them with:

  • Managing their medications
  • Lifestyle support
  • Psychological support
  • Blood tests
  • Electro-cardiograms (recording electrical activity in the heart)

We aim to provide care for all patients who are diagnosed with heart failure. We work closely with other professionals to improve the outcomes for patients with heart failure, and reduce hospital stays.
