Heart failure

The Sandwell and West Birmingham acute heart failure team consists of six Heart Failure Clinical Nurse Specialists and one admin support who work across Sandwell and City Hospitals.

They work alongside the community heart failure team, based at The Lyng, in West Bromwich, who hold regular clinics within GP surgeries and also see patients at home.

Team

The Heart Failure team, both acute and community, provide patient support and education on coping with their diagnosis. 

 

Services

We provide specialist nursing for the management, support and education of patients with chronic heart failure. We work closely with cardiologists (heart specialists) and lead physicians, treating patients at all stages of coping with heart failure.

Within the community, we care for patients with a combination of home visits and telephone contact. We also provide community clinics where patients are seen in a general practice or health centre by a specialist nurse.

Both the acute and community service  assess patients and help them with:

  • Managing their medications
  • Lifestyle support
  • Psychological support
  • Blood tests
  • Electro-cardiograms (recording electrical activity in the heart)

We aim to provide care for all patients who are diagnosed with heart failure. We work closely with other professionals to improve the outcomes for patients with heart failure, and reduce hospital stays.
