Children’s Community Nursing

The Community Children’s Nursing (CCN) service works together with GPs, paediatricians, hospitals, health visitors, school nurses, social workers and voluntary agencies to enable children with acute and chronic conditions to be cared for at home and school.

The team is made up of eight different services:

  • Continuing Care Team
  • Paediatric Virtual Ward
  • Paediatric Continence Team
  • Paediatric Diabetes Team
  • Paediatric Epilepsy Team
  • Paediatric Virtual Ward
  • School Educational Needs Team
  • Short Intervention Chronic Conditions

Virtual ward aims to support earlier hospital discharges and prevent hospital admissions. We have different pathways that we can take children onto which are:

  • Asthma/viral induced wheeze
  • Bronchiolitis
  • Jaundice
  • Diarrhoea and Vomiting
  • Eczema
  • Disimpaction regime with constipation
  • Nephrotic Syndrome
  • Tonsillitis

    What to do out of hours

    Out of hours, if you are under the care of virtual ward currently please ring the Paediatric Assessment Unit on 0121 507 3266 or if you are concerned that your child is unwell attend A&E.
    If you are concerned about your child and not currently under the virtual ward team please contact your own GP, 111, or attend A&E.

    How to refer

    We take referrals from the children’s wards at Sandwell General Hospital, Children’s Emergency Care Unit at City Hospital, from the clinical nurse specialist teams and from paediatric consultants.
    We have a referral form that is completed by the nursing/medical teams to refer to us and this is sent to the generic email address.

Our aim is to promote healthy bladder and bowels for children aged 4–19 years, who live or go to school in the Sandwell area. We aim to maximize children’s independence with toilet skills within their personal capability.

What to do out of hours

Families need to contact either their GP, urgent care provider, NHS 111 or go to A&E if they have any concerns out of the service hours.

How to refer

We do not accept self-referral from families, referrals need to be made by health professionals working with the child. If you are a health professional, please complete our service referral form available on the Trust intranet.

The Roald Dahl Marvellous Childrens Charity provide specialist nurses and support for seriously ill children by working in partnership with the NHS. There are currently over 80 Roald Dahl Specialist Nurses caring for over 24,000 seriously ill children across the UK.
Our clinical nurse specialists offer support, information and advice to families and liaise with other professionals. As clinical experts they can answer questions about your child’s condition, work closely with the consultants and local services, monitor and adjust treatments.

What to do out of hours

For urgent medical advice, contact your GP or NHS 111. For medical emergencies call 999.

How to refer

Your child’s GP or Paediatrician should make the referral to the epilepsy service. A referral may also be made after your child is admitted to the Hospital or from another Hospital.

The short intervention chronic care team provide support to patients with long term, chronic conditions and their families. They provide training for families and schools, end of life care and support for palliative patients. Additionally, the team see acute patients in the community to complete treatment post discharge and facilitate referrals to other teams within the MDT

What to do out of hours

Out of hours referrals will be picked up during working hours. Patients under the CCN team will have open access to Sandwell Paedicatic wards out of CCN hours. They need to call the ward before arriving.
