Patients who have an outpatient appointment will receive details via a text message on their mobile phone as part of a newly introduced online appointment system.
This means they are able to cancel or change their appointments by using the online portal via an app which can be downloaded onto their smart device. More details about the app can be found here: https://www.swbh.nhs.uk/patients-visitors/outpatients/online-appointment-system-outpatient-portal/
Those patients who have opted out of using this system can cancel or amend their appointment by calling 0121 507 4151 or by downloading and completing the form below and returning it via email to swbh.cancelmyappointment@nhs.net
Please allow two working days for a response from our team. If you would like to amend an appointment for the same day please call the relevant telephone number on your letter alternatively you can complete the form below.
Please note, if you are unable to attend because you are on holiday, please ensure you provide us with the dates you are away.
Completed forms should be returned by email to swbh.cancelmyappointment@nhs.net
Please give as much notice as possible so we can offer your original appointment slot to someone else.
Patients may cancel a new appointment on one occasion, however if it is cancelled a second time, they will be referred back to their GP for a new referral. General enquiries will NOT be responded to.
