The key role and responsibilities of the Primary Care Liaison Service is to be the named lead for the Trust for resolving issues, connecting departments and improving the interface with Primary Care and other partners across ICS Systems.
They are here to offer support and guidance to all our system partners.
By working in partnership, we will ensure that our patients have access to the best possible care and reduce workload impact for our system partners.
