District nursing

District nurses are a team of highly skilled nurses; we carry out a wide variety of services, and provide a community based service for patients with a physical healthcare need.

The service includes visits to housebound patients who need nursing treatment. Depending on our patients’ needs, we make referrals to other agencies for additional treatment where required or appropriate.

Team

Our community nurse managers are Tina Jackson and Louise Johnson.

There are teams of district nurses right across Sandwell, including:

  • Warley/Whiteheath
  • Mace St/Rowley
  • Glebefields
  • Victoria/Sherwood
  • Cape Hill/Rood End
  • Oldbury
  • Cross Community Nurses
  • Mesty District Nursing Team
  • Neptune District Nursing Team

We also provide an Out of Hours District Nursing Service and a Community Practice Teachers Team.

Services

Working from a GP surgery, healthcare centre, or directly in patients’ homes, district nurses provide care for patients aged 16 and upwards. Treatments include:

  • Treatment of wounds, leg ulcers or pressure sores
  • Continence advice and assessment
  • Intravenous therapy
  • Blood tests
  • Stop smoking advice and referral on to appropriate service
  • Removal of stitches and clips
  • Catheter management
  • Holistic assessments (overall physical and mental wellbeing)
  • Health education
  • Palliative care (coping with on-going conditions)
  • End of life care
  • Injections

District nurses also provide leg ulcer care for patients in health centres and clinics across Sandwell. If you need to see another healthcare professional, your district nurse can refer you.
