Are you aged between 12-18 yrs old?

Can you help the Children & Young People’s board, from our Trust, design a poster which aims to help raise awareness about the need for young people with long term medical conditions to complete the ‘Ready Steady Go Transition tool’.

What is the Ready Steady Go Transition tool?

This is a checklist for young people, who are aged between 14-16 years, and suffer from conditions such as diabetes, asthma and Epilepsy. It should be completed so doctors and nurses can assess if they are ready to have their care transferred from the children’s to the adult’s department.  It helps the doctors and nurses assess whether the patient knows enough about their condition and how they can look after themselves.

Why do we need the poster?

We want to encourage more young patients and their families to complete the checklist and we  hope the poster will help remind them to do this. 

What to do if you want to take part in the competition:

We would like you to redesign or improve the current poster. It will need to include the word ‘transition’, and have a similar explanation to what it means as featured on the current poster. It should also refer to the ‘ready, steady, go’ checklist. The hospital logo will also be on it. The rest of what it says and what it looks like is up to you!

 What is the prize?

The winner will win £50 Amazon voucher. The winning poster will be enlarged and be put up in the children’s clinical area.
