Our award winning Integrated Care Service (ICARES) is a service and innovative approach to managing adults with long term conditions irrespective of their diagnosis, location or age. Here, you’ll work with a whole range of dedicated professionals from Elderly Care & Primary Care Co-ordinators and Home Accident Prevention Officers to Rehab Support Workers and Alcohol Case Management, to Therapists, Physiotherapists and Clinical Leaders. Together, you’ll provide specialist community interventions to avoid unnecessary admissions to hospital, help maintain health and well being, and improve independence and function. The iBed team offers the best of both worlds – integrated community services in a hospital setting. You’ll work closely with GPs and social workers, so we’ll look to you to take the lead on patient care. The community hospital bed base has grown a lot over the last couple of years, so have the opportunities to specialise and focus your career development. Whatever you’re truly passionate about, we’ll bring your ambitions to life. You can choose to work across all three locations in rotation or be primarily based at one site. It’s up to you. We’re flexible and want to find out what works best for you. Across three wards, you’ll focus on holistic nursing. Working in a multi-disciplinary team, you’ll take a softer approach building relationships with older patients, achieving functional recovery rather than relying solely on drugs and procedures. You’ll have an excellent support network with coaching and guidance from passionate Band 6 Nurses. The jewel in the crown of Communities & Therapies, this purpose built site has 20 single rooms which has adopted the travel lodge model. Here, you’ll help patients transition from acute to convalescent care. We have two end of life beds which means you’ll be able to work closely with hospices and provide thoughtful palliative care as well. Set in a great location, we have two refurbished wards – MedFit and Rehab. If you’re looking for a rewarding challenge, this is it. Working closely with Social Care, you’ll get each pathway right. The scheduled care team encompasses the management of both acute and chronic conditions in a vast variety of areas such as endoscopy, outpatients, medical day case, theatres and alongside our community care colleagues. The Scheduled care team work tirelessly with various members of the multi-disciplinary team to provide high quality care and avoid unnecessary hospital admissions. Together, you’ll provide specialist treatment plans, education in self-management of conditions and work closely with our research colleges to advance knowledge of medical conditions and create new treatments. Critical Care provides excellent support to band 5’s looking to enhance their career by gaining experience in acute Care. We have a structured development programme led by our Professional Development nurse and strong clinical band 7 tier .New starters receive 6weeks supernumerary time fully supported by excellent mentors. Once completed you will commence your module training which takes care of your first two years training and development. We provide Critical Care across two sites but work as one service with all staff rotating within a professional team to ensure your support follows you .In addition we have an outstanding Critical Care Outreach and Follow Up Support Service. As a theatre Practitioner you will provide high quality patient centred care reflecting the beliefs outlined in the Theatre philosophy. The practitioner will act as an advocate for patients. You will rotate within theatre to gain experience in all theatre areas including Scrub and Recovery with the opportunity to gain a qualification in Anaesthetics. You will get the opportunity to rotate cross-site as well as by specialty on a planned basis. We are fully committed to your staff development with a structured professional development team within Theatres. You will participate in elective surgery for all specialties excluding Cardiology. Our acute paediatric unit currently consists of an inpatient ward with HDU capacity, an adolescent unit and two Paediatric Assessment Units that are presently positioned on two sites (Sandwell Hospital and City Hospital). We also deliver outpatient and medical and surgical day case activity on both sites. There is a planned rotation, with supporting competency outcomes, for all staff to allow them the opportunity to develop in all areas of acute paediatric care. All of our services will move to the brand new Midland Metropolitan University Hospital (MMUH, opening in 2024): a hospital where superheroes are not only the theme within our paediatric unit but the way we view every member of staff within our team. Sandwell and West Birmingham NHS Trust is proud to be striving towards being recognised as the best integrated Trust within the West Midlands, with our paediatric directorate being made up of both acute and community teams, inclusive of children’s therapy services for community. This integrated service delivery facilitates the development and improvement of pathways between community and acute services to provide seamless care for children and their families.
